Attached to this post are four helpful documents from Medicare....

These documents were sent by Carol Hanson and Sharon Johnson, Health Insurance Specialists from CMS Medicare out of Chicago, ahead of a seminar which they are giving to the members of the Workers Compensation Section of the State Bar of Michigan at Boyne Highlands Resort on Friday, June 18.

Of particular interest is the checklist and the power point presentation in .pdf format. The checklist is the information that CMS needs from the attorneys to evaluate and approve a medicare waiver or set-aside trust. The Power Point presentation provides the legal basis for the work that CMS does and is a summary of its procedures.
